WHAT DOES IT FUND? The purpose of this program is to help communities develop capacity for, expand, deliver, and sustain evidence-based chronic disease self-management education (CDSME) and support programs that address the prevention and management of chronic conditions among older adults and adults with disabilities. Click here for more information. 

WHO'S ELIGIBLE? Local and state governments, special districts, public housing authorities, Indian housing authorities, institutions of higher education, nonprofit organizations, faith based organizations, community organizations, and Native American tribal governments.
